Once your copy of the file is open, you can tab through from one field to the next. You can fill in the fields using your keyboard and print the result out for your signature and submission to our office.

Note to parents of Alta Regional Center Kids:

Order of application for Ride To Walk if your child is with ALTA REGIONAL CENTER:

1. Fill out the Rider Application, online at www.ridetowalk.org. Mail completed application to Sierra Pediatric Therapy Clinic, 8485 Barton Road, Granite Bay, CA 95746, along with a copy of your insurance card.

2. Ask your ALTA caseworker to issue a 1 hour POS for an evaluation for therapeutic riding.

3. Sierra Pediatric Therapy Clinic will call you to schedule an evaluation for your child when they have your completed application and POS for the evaluation from Alta.

4. Evaluation is then sent to your caseworker, who then issues a 5 hour POS for riding.

5. Parent/Guardian required to attend Parent/Volunteer Training class held the first Saturday of every month at Ride To Walk from 1:00 - 3:00 p.m.

6. NOW, we can schedule the rider to ride.
